This is from my 1975 D-35 when purchased new. Booklet is 9" x 3.75" and in fair to good condition. There are age spots, a slight tear in the cover fold, the stables holding it together are rusted. Cover is rough but inside pages are fairly pristine. Date and place of purchase (Hilo, Hawaii) and purchase price ($550!) are visible on the first page. There's no copyright date suggesting the publishing range, but the pictures show a dreadnaught guitar of the sixties with its telltale, rounded peghead, so the booklet was probably used earlier than 1975, too.
