It's in a tri-fold cardboard case with a jewel insert that holds the CD in place. Autographs are on the CD itself and on the inside cover of the case, in multi-color sharpie. Bottom right corner of cover looks like it had some water on it at some point and the paper is starting to peel there (see pics) - only on the front cover, not on the rest. Saw them live at Scarborough Fair (Ren-Fest) in Waxahachie, TX several years in a row ~2010. Got this CD in-person at one of their shows, and got them to all sign it afterward.
