After he unexpectedly passed in 2000, a benefit show was held in May, featuring TSOL, X, Agent Orange, The Offspring, Pennywise, Punk Rock Karaoke, and Social Distortion/Mike Ness acoustic. It was at this show where these cards were distributed. I went to the show and held on to one of these!
