Description: This book represents the distillation of a tremendous volume of literature, filtered through the receptive but, I hope, discriminating awareness of a single author. When I wrote the first edition in 1973, the task was challenging mainly because few had tried a synthesis of what was then known. But—as most who read this book are well aware—in the ensuing 20 years, the task has become much more difficult, mainly because the literature of hypertension has grown so that it is almost beyond the grasp of any one person. I continue to be a single author (with the important exception of the chapter on children written by Dr. Ellin Lieberman) for these two reasons: • First, a single-authored text should offer more cohesion and completeness and, at the same time, brevity and lack of repetition, in comparison to the usual multi-authored but rarely edited mega-book. • Second, I have the time, energy, and interest to keep up with the literature, and this book has become the major focus of my professional life. The success of the previous editions and the many compliments received from both clinicians in the field and investigators from the research bench have prompted me to do it again. The very reason that a single-authored text has become such a rarity explains the need for a new edition fairly often. As with previous editions, almost every page has been revised, using the same goals: • Give more attention to the common problems. Primary (essential) hypertension takes up almost half of the book. • Cover every form of hypertension at least briefly, providing references for those seeking more information. Additional coverage is provided to some topics that are unusual but more interesting because new insights about them have become available. An example is glucocorticoid remediable aldosteronism, the genetic basis of which has been uncovered. • Include the latest data, even if available only in abstract form. • Provide enough pathophysiology to permit sound clinical judgment. • Be objective and clearly identify biases. Although my views may counter those of others, I have tried to give reasonable attention to those with whom I disagree. At the request of readers, the full references are now provided, rather than the abbreviated ones in previous editions. This takes up more space, so to limit the length of the book, some of the original and multiple subsequent sources have been deleted in favor of more recent and inclusive references. Some truly classic references have been kept, but I apologize to authors whose original contributions have been preempted by references to subsequent work of others. Once again I am pleased that Dr. Ellin Lieberman, Head of Pediatric Nephrology at the Children's Hospital of Los Angeles, has contributed a chapter on hypertension in children and adolescents. I am particularly indebted to Dr. William Neal for the many hours spent in proofing and organizing the content. I have been fortunate in being in an academic setting wherein such endeavors are nurtured, much to the credit of my former chief, Dr. Donald Seldin, and my current chief, Dr. Daniel Foster. I have been greatly helped by my secretaries, Diana Dubois and Sharon Washington. And lastly, the forbearance of Audrey, my wife, can only be acknowledged by the promise that I will not do it again-at least for another four years.
